Questionnaire on behalf of the Complainant / Petitioner Smt. Rama Dhar, to Dr. Amitabha Ghosh.

 

Question no.1.      Did you received any notices from the Hon’ble District Consumer Disputes Redressal Forum, Alipore, South 24 Parganas ?

Question no.2.      Did you enclose the request letter of Dr. Sumit Gulati and his team of doctors ?

Question no.3.      How come you feel and placed and planned yourself as an Expert, in the present case matter ?

Question no.4.      This is true that Dr. Sumit Gulati and Dr. Kallol Banerjee, jointly hired you to put as dictated by them, say yes or no, if no, state the true story.

Question no.5.      Who seeks your perverted opinion as an expert ?

Question no.6.      How did you received the copy of the Complaint, written version and evidences of all the parties along with the treatment papers filed before the Hon’ble Forum ?

Question no.7.      Can you say on which date, Dr. S. Gulati, Dr. Kallol Banerjee, and Dr. Debasish Roy, filed their Evidence on Affidavit, before the Hon’ble Forum ?

Question no.8.      How much fees you have taken from Dr. S. Gulati and Dr. Kallol Banerjee ?

Question no.9.      state the name of your Learned Advocate, under who’s instructions you acted upon ?

Question no.10.    What is difference between opinion and expert opinion ?

Question no.11.    What is meant for “team of Doctors” ?

Question no.12.    State the date, place and time of visit at you by them ?

Question no.13.    Dr. Kallol Banerjee, is aged about 49 years, as stated by him, Dr. Debasish Roy, aged about 58 years, as stated by him, and Dr. S. Gulati, aged about 39 years, as stated by him, then how come at the age of 48 years, you are posing yourself as an expert, while you are younger to them and have less experience as observed due to the ages, and others, state the described reasons behind that ?

Question no.14.    Did you ever find in the Written Version and or in the Evidence on Affidavit submitted by the Doctors ( i.e. Dr. S. Gulati, Dr. Kallol Banerjee, and Dr. Debasish Roy ), that they have described, agitated, and or prayed about and or for your any opinion in any manner whatsoever ?

Question no.15.    Did you ever place any application before the Hon’ble District Consumer Disputes Redressal Forum, Alipore, South 24 Parganas, that you want to place and or produce your any opinion in favour of the doctors (i.e. Dr. S. Gulati, Dr. Kallol Banerjee, and Dr. Debasish Roy ) ?

Question no.16.    Are you engaged in any Government Hospital, state the name of such Hospital, and duration of your such engagement ?

Question no.17.    Did the State Medical Council recognized you as an expert, if yes, then placed the relevant documents and or papers, and if no, then how did you place yourself as an expert, states and describe ?

Question no. 18.   What is the parameter which determine as an Expert ?

 

Question no.19.    It is true that Dr. S. Gulati, Dr. Kallol Banerjee, and Dr. Debasish Roy, are your friend and all of you know each other, very well.

Question no.20.    Did you present and or participate in treatment and or in operation of the Complainant ?

Question no. 21.   Did you have any knowledge of all medicine and tests and the reports causes on the Complainant, if yes, please describe with full details and particulars thereof ?

Question no. 22.   Did you ever visit the Court premises ?
